1. Telehealth Services
Telehealth involves using electronic communications to share patient medical information among healthcare providers to improve patient care. These services may include remote monitoring, prescription refills, and appointment scheduling.
- Benefits: Enhanced access to care, efficient medical management, and the expertise of specialists.
- Risks: Potential delays due to technical issues, incomplete medical records, or breaches of privacy despite our security protocols.
By using our telehealth services, you agree to:
- Accept these services as appropriate for your care, determined by our providers.
- Acknowledge the potential for technical limitations or risks.
- Understand that telehealth services complement but do not replace in-person primary care.
2. SMS Terms and Conditions
By providing your phone number, you consent to receive SMS messages from The Hope Wellness Center regarding appointments, account updates, and service follow-ups.
- Message Frequency: 2-3 messages per appointment.
- Opt-Out: Reply “STOP” to opt out at any time. To rejoin, reply “START.”
- Message and Data Rates: Standard rates may apply based on your mobile carrier.
- Privacy: Your data is protected according to our Privacy Policy.

3. Privacy and HIPAA Compliance
We adhere to HIPAA regulations to ensure the privacy of your Protected Health Information (PHI). PHI may be shared only as necessary for your care, including with other healthcare providers, labs, or insurance companies.

- Requesting restrictions on PHI usage.
- Accessing your medical records in compliance with state and federal laws.
- Reporting privacy concerns to our office manager.
4. Financial Policies
- Copayments and other fees are due at the time of service.
- Missed or late cancellations (less than 24 hours’ notice) incur a $50 fee.
- Self-pay patients are billed according to our standard rates.
- Telehealth patients are required to keep a credit card on file for billing purposes.

5. Cancellation and Refund Policy
- Missed or late cancellations result in a $50 fee.
- After three missed appointments in 90 days, services may be terminated.
- Refunds are offered if you are unsatisfied with the services provided.
6. Use of Electronic Communication
By providing your contact information, you consent to electronic communication for scheduling, reminders, and other service updates.
- You may request paper copies of electronic communications for a fee.
- Ensure your contact information is current to avoid disruptions in communication.
7. Consent for Treatment
By receiving services from The Hope Wellness Center, you agree to:
- Voluntarily participate in mental health treatments, including therapy and medication management.
- Understand that no specific results are guaranteed.
- Provide accurate health history and disclose any changes to your condition.
8. Emergency Services
The Hope Wellness Center does not provide emergency services. If you are experiencing a medical or psychiatric emergency, call 911 immediately or contact the Suicide Prevention Lifeline at 1-800-273-8255.
9. Limitation of Liability
The Hope Wellness Center is not liable for:
- Technical failures during telehealth services.
- Issues arising from the use of third-party platforms.
- Losses or damages resulting from the use of our website or services.
10. Updates to Terms and Conditions
We may update these Terms and Conditions periodically. Changes are effective immediately upon posting. Continued use of our website or services constitutes acceptance of the updated terms.
